12-Month Claims Data Shows Rimegepant Delivers Greater Real-World Persistence for Acute Migraines

When treating migraine attacks as they happen, finding a medication that works is only half the battle—patients also need a treatment they can consistently rely on without disruptive side effects.

A landmark retrospective cohort study published in Cephalalgia provides the first real-world observational data comparing how long patients actually stay on two newer acute migraine therapies: rimegepant (a gepant) and lasmiditan (a ditan). The results were clear: patients taking rimegepant were significantly more likely to continue their treatment over a 12-month period.

Both rimegepant and lasmiditan represent advanced, targeted mechanisms of action designed for patients who cannot tolerate or do not respond to traditional triptans. How the Study Was Done

Researchers analyzed de-identified US insurance claims from the massive MarketScan databases, identifying adults who newly initiated treatment with either rimegepant or lasmiditan between March 2020 and December 2022.

  • Overcoming a History of Controlled Substances: The researchers also looked at a subgroup of patients with a history of using controlled substances (like opioids or butalbital) or central nervous system (CNS)-active nausea medications for their headaches. Because lasmiditan is a Schedule V controlled substance with known sedative properties, past behaviors could theoretically influence how patients use it. However, rimegepant consistently came out on top, showing significantly higher persistence rates regardless of whether patients had a history of repeated controlled substance use.

Why the Large Gap in Persistence?

The study authors note that real-world treatment persistence is heavily driven by a patient’s total experience, balancing speed of relief against side effects that hinder their ability to function. The massive gap between these two drugs likely comes down to everyday lifestyle barriers:

  • The 8-Hour Driving Restriction: Lasmiditan carries a strict FDA label restriction requiring patients to avoid driving or operating machinery for at least 8 hours after every single dose due to risks of incoordination. This frequently forces patients to delay taking their medication until they are safely home, violating the gold standard clinical rule of treating a migraine immediately at onset. Rimegepant carries no such driving restriction.
  • Adverse Event Profiles: Lasmiditan is associated with central nervous system side effects like dizziness, sedation, and fatigue in more than 5% of users. In contrast, rimegepant’s most common side effect is mild nausea, occurring in only 2% of users, allowing patients to treat their pain and seamlessly return to their daily routines.
